Our Assertive Outreach Teams offer a commitment to supporting people with psychosis and unusual distressing experiences. We support people who have had frequent admissions to hospital and have other complex recovery needs.

Our team members are a skilled and experienced multi-professional team working with people who would benefit from ongoing support in the community. We work in partnership with the individual, their families, friends and carers and also alongside other agencies, to support recovery and wellbeing.

We offer flexible and meaningful interventions that aim to support patient’s recovery and enable you to become more independent.

The support we offer can be frequent and ongoing – people using the service can get support every day if needed, but we aim to work towards independence. Working together could include illness related matters (e.g. medication) but also may include working on social issues, such as finances, benefits, housing and accessing education and employment services.
