We provide specialist support and treatment to older adults experiencing moderate to severe mental health problems. We also support people who have a diagnosis of Dementia who have complex and challenging needs relating to this. We offer support with both health and social care needs.
Our teams are made up of professionals including Community Psychiatric Nurses (CPN’s), Social Workers, Care Managers, Occupational Therapists, Psychologists, Support Time Recovery Workers and Psychiatrists.
There are a number of different teams that makeup the SOAMHS services, including:
- Enhanced Duty Team (ENDU), which provides short term intensive support, treatment and interventions to people with a diagnosis of Dementia who have urgent complex needs
- Care Home Inreach Team, who work in partnership with care and nursing homes to improve the quality of life of people with dementia
- Older people mental health liaison, providing high quality care for individuals who have healthcare needs related to dementia, or older adults with specialist needs as a result of complex mental health problems
- Memory Assessment Service/ Dementia Assessment Service, who provide early detection, diagnosis, treatment and care for people who have problems associated with dementia
