Beyond the Medicine: Meet the Pharmacy Professionals Improving Mental Health Care Every Day
Grant Salvage - Lead Pharmacist & Independent Prescriber,
Chichester Inpatient & Urgent Care Services
"When most people think of a pharmacist, they still too often picture someone behind a community counter dispensing prescriptions. In specialist mental health care, however, the role extends far deeper into direct, hands-on clinical practice. As a Lead Pharmacist and Independent Prescriber within the Chichester locality, my day-to-day work focuses on delivering bespoke, evidence-based treatments that resolve acute crises, support long-term recovery, and keep care rooted in the community.

A day in the life: from inpatient wards to crisis care
My role spans the full spectrum of acute mental health services, bridging the gap between inpatients in Oaklands Ward and emergency community support with the Crisis Resolution and Home Treatment Team. On any given morning, you will find me participating in multidisciplinary ward rounds, reviewing complex medication regimens for conditions such as mood disorders, psychotic illnesses, severe anxiety, and trauma-related conditions, and advising on the most appropriate treatment adapted for the individual that is most likely to resolve the cause of their admission and reduce the likelihood of future admissions.
By the afternoon, my focus shifts entirely to my role as an Independent Prescriber embedded directly within the crisis team. I run dedicated prescribing clinics, conduct face-to-face consultations, and undertake home visits for individuals experiencing acute mental health crises. Rather than simply advising on medication, I directly initiate, adjust, and optimize complex treatment regimens in real-time utilising the skills, knowledge, and expertise in the use of psychiatric medications that I have developed in our services over the past 12 years. Whether managing rapid psychotropic titrations or tailoring bespoke treatment plans, I work alongside crisis clinicians and patients in their own homes to stabilise acute symptoms quickly, manage side effects, and provide immediate, compassionate care when patients need it most.
Breaking the cycle: avoiding hospital admissions through collaboration
True mental health care relies on seamless continuity between hospital wards, urgent care services, and community teams. One of the most vital aspects of my role is expanding access to in depth medication reviews within the community and acting as a point of continuity between our inpatient, urgent care, and community services to assist in resolving crises before they lead to hospital care.
Every week, I also join multidisciplinary colleagues to participate in a collaborative clinical review of the Chichester and Bognor locality bed list. By evaluating each patient's clinical needs together, we identify who can be safely and effectively supported in the community. This collaborative approach ensures hospital admissions are used only when necessary and have clear purposeful aims that can only be completed in the hospital environment, helping individuals remain close to their home and support networks.
Collaborative expertise across the trust
Specialist pharmacy is built on partnership. Alongside my core crisis and acute inpatient work, I provide expert psychopharmacology advice across secure forensic beds at The Chichester Centre, rehabilitation services at Connolly House, and liaison teams.
Working alongside psychiatrists, nurses, therapists, and patients, we ensure medicines are used as an empowering tool for recovery, proving that modern pharmacy is truly about treating the whole person, far beyond the medicine."
